Mechanism and pharmacology

Natriuretic peptide receptor-A (NPR-A) agonist. Binding to NPR-A on vascular smooth muscle and endothelium increases cGMP, causing vasodilation, natriuresis, and RAAS suppression. Reduces preload and afterload in heart failure.

Nesiritide (Natrecor) was FDA-approved in 2001 for acutely decompensated heart failure, but US marketing was discontinued in 2018. What are nesiritide's main safety signals?

The most significant safety signal is hypotension, occurring in 26.6% of patients in ASCEND-HF vs 15.3% placebo. Serum creatinine elevation was observed in some studies.
