Mechanism and pharmacology

Guanylate cyclase-C (GC-C) agonist. Analog of uroguanylin, an activator of GC-C. Binds to GC-C on intestinal epithelial cells with pH-dependent affinity (optimal at pH ~5). Increases intracellular cGMP, activates CFTR, and promotes chloride/bicarbonate-rich fluid secretion. Preferential activity in the proximal small intestine compared to linaclotide due to pH-dependent binding. Not measurably absorbed orally.

Plecanatide (Trulance) is a 16-amino-acid uroguanylin analog and guanylate cyclase-C agonist with two disulfide bridges. It has pH-dependent binding with optimal activity around pH 5, which may confer preferential activity in the proximal small intestine. It is FDA-approved for chronic idiopathic constipation and IBS-C but not approved in the EU.

No. Both are GC-C agonists but plecanatide is a 16-amino-acid uroguanylin analog with two disulfide bridges, while linaclotide is a 14-amino-acid peptide with three disulfide bridges. Plecanatide has pH-dependent binding (optimal at pH about 5) with preferential activity in the proximal small intestine compared to linaclotide.

What are plecanatide's main safety signals?

Plecanatide is contraindicated in pediatric patients under 6 years (animal studies showed dehydration deaths) and should be avoided in patients 6 to 17 years. The most common adverse event is diarrhea (about 5% on drug versus 1% placebo). It is contraindicated in mechanical GI obstruction. No long-term safety data beyond 1 year in controlled trials.

Is plecanatide prohibited in sport?
